ForgeMeds, acting as a Business Associate, and our affiliated independent Medical Groups and Pharmacies (collectively "We") are required by law to maintain the privacy of your Protected Health Information (PHI). We must provide you with this notice about our privacy practices, our legal duties, and your rights concerning your PHI.

II. How We May Use and Disclose Your PHI

• Treatment: We may use or disclose your PHI to physicians, nurses, pharmacies, and other healthcare personnel who provide you with healthcare services.
• Payment: We may use and disclose your PHI to obtain payment for services we provide to you, including processing Stripe transactions and issuing receipts.
• Healthcare Operations: We may use and disclose your PHI in connection with our healthcare operations, including quality assessment, clinical guidelines review, and customer service compliance.
• As Required by Law: We will disclose your PHI when required to do so by federal, state, or local law.

III. Your Health Information Rights

• Right to Inspect and Copy: You have the right to inspect and copy PHI that may be used to make decisions about your care. You can access most of this directly via your ForgeMeds Client Dashboard.
• Right to Amend: If you feel that PHI we have about you is incorrect or incomplete, you may ask us to amend the information.
• Right to an Accounting of Disclosures: You have the right to request a list of certain disclosures we made of your PHI for purposes other than treatment, payment, and healthcare operations.
• Right to Request Restrictions: You have the right to request a restriction or limitation on the PHI we use or disclose about you for treatment, payment, or healthcare operations.
