I. Acceptance of Terms

By accessing or using the ForgeMeds website, platform, and related services (collectively, the "Platform"), you agree to be bound by these Terms of Service. If you do not agree to these terms, you may not use our Platform. ForgeMeds reserves the right to modify these terms at any time, and such modifications shall be effective immediately upon posting.

II. Nature of Services & The Platform

ForgeMeds is a technology company, not a healthcare provider. We provide a digital Platform that connects you with independent, physician-owned medical affiliate groups (the "Medical Groups") and independent, licensed 503A or 503B compounding affiliate pharmacies (the "Pharmacies").

ForgeMeds does not provide medical advice, diagnosis, or treatment. All clinical decisions, including the decision to prescribe medication, are made solely by the licensed healthcare providers affiliated with the Medical Groups. Your relationship with the healthcare provider is independent of your relationship with ForgeMeds.

III. Medical Emergencies

DO NOT USE THE PLATFORM FOR MEDICAL EMERGENCIES. IF YOU ARE EXPERIENCING A MEDICAL EMERGENCY, CALL 911 OR GO TO THE NEAREST EMERGENCY ROOM IMMEDIATELY.

The Platform is designed for routine, non-emergency consultations regarding specific, discrete conditions. It is not a substitute for your primary care physician.

IV. Prescriptions & Compounded Medications

There is no guarantee that you will receive a prescription. Prescriptions are issued only if a licensed provider determines it is clinically appropriate after reviewing your health assessment and conducting a consultation.

• Pharmacy of Choice: You have the right to fill your prescription at any pharmacy of your choice. However, the Platform is designed to seamlessly route prescriptions to our vetted affiliate Pharmacies for your convenience and to fulfill the subscription model.
• FDA Disclaimer: You acknowledge that compounded drugs are formulated specifically for individual patients. While the active pharmaceutical ingredients used may be FDA-approved, the final compounded medications themselves are not FDA-approved as finished products.

V. Subscriptions, Billing, and Cancellations

Certain products and services on the Platform are offered on a subscription basis. By enrolling in a subscription, you authorize ForgeMeds (via our third-party payment processor) to automatically charge your provided payment method on a recurring monthly basis.

• Inclusive Pricing: Subscription fees typically cover the cost of the medical consultation, the medication (if prescribed), and standard shipping.
• Cancellation Policy: You may cancel your subscription at any time via your Client Dashboard. To avoid being charged for the next billing cycle, you must cancel before your next scheduled billing date. Once a pharmacy has processed and dispensed a prescription, that specific order cannot be refunded (see our Refund Policy).

VI. User Responsibilities & Accuracy of Information

You agree to provide accurate, current, and complete information about your medical history, current medications, and physical condition. Providing false or incomplete information can severely impact the safety and effectiveness of your treatment. You agree to update your medical profile promptly if your health status changes.

VII. Limitation of Liability

To the fullest extent permitted by applicable law, ForgeMeds and its affiliates, officers, employees, and agents shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Platform, including but not limited to any medical outcomes or adverse reactions resulting from treatments prescribed by the independent Medical Groups.
