Patient Bill of Rights

I. Respect and Non-Discrimination

You have the right to considerate, respectful care from all ForgeMeds personnel and affiliated medical providers at all times and under all circumstances, with recognition of your personal dignity. You have the right to receive care free from discrimination based on race, color, religion, sex, national origin, disability, or sexual orientation.

II. Information and Informed Consent

You have the right to obtain from your healthcare provider complete, current information concerning your diagnosis, treatment, and expected outcomes in terms you can reasonably be expected to understand. You have the right to receive information necessary to give informed consent prior to the start of any procedure or treatment.

III. Refusal of Treatment

You have the right to refuse treatment to the extent permitted by law and to be informed of the medical consequences of your action. Should you refuse recommended treatment, the affiliated Medical Groups may terminate their professional relationship with you upon reasonable notice.

IV. Provider Identity

You have the right to know, by name and specialty, the physician or clinical provider responsible for coordinating your care via the telehealth platform.
